The value-based care model encompasses six primary elements. The goal is to reduce healthcare spending and boost patient outcomes for patients with chronic kidney disease or end stage kidney disease.
By networking our clinical practices with Quality Care Analytics, VKC can implement appropriate preventative and early treatment measures. We are working to shift away from event-based and crisis-driven healthcare to a holistic support model across the patient care life span.
