We provide 24/7 Remote Patient Monitoring as well as real time communication channels to patients with their providers, which has resulted in significant reduction in unnecessary hospitalization.

We have in house educators to counsel patients & also have validated tools such as P.A.M to measure the knowledge, skills and confidence of our patients in managing their own wellbeing.

Increase Transplants

We have seen an uptick in transplants because of our collaboration with kidney foundation and engagement with multiple transplant facilities including Stanford, UC Davis, UCSF (pre & post clinic in-house and CPMC.

Utilize Technology
